High School Homeschool Transcript Checklist
1. Student Information
Student’s full legal name
Date of birth (optional)
Graduation date (month/year)
Homeschool name (can be “Home Education Program” or family name)
City, state, and country
Parent/guardian name (as administrator)
2. Course List (Grades 9–12)
For each academic year, list:
Grade level (9th, 10th, 11th, 12th)
Course title
Subject area (Math, English, Science, etc.)
Final grade (letter or percentage)
Credits earned
Tip: One full-year course = 1.0 credit (about 120–180 hours).
Semester course = 0.5 credit.
3. Core Subject Requirements
Most colleges expect to see:
